How do I delete a questionnaire?
You can delete any questionnaires that you or other users have created by locating these and selecting 'delete'.
You are in charge of your own data and therefore have the ability to delete your questionnaires if they are no longer required. To delete a questionnaire you have to ensure no 'accepted' shares are attached and deny these beforehand.
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. Use the search box in your dashboard to locate the questionnaire you wish to delete
3. View the amount of shares attached to the questionnaire on the questionnaire tile
4. If at least one share is shown, select 'Share' first to deny this
5. Select 'Deny' for any sharing currently in the 'Accepted' or 'Pending' status
6. Confirm your choice by selecting 'Yes'
7. Now that any sharings have been denied, return to your dashboard by selecting 'Dashboard' at the top of the screen
8. Locate the questionnaire you wish to delete by using the search box in your dashboard
9. Once located, select 'Delete'
10. Confirm that you want to delete your questionnaire

How do I start a questionnaire?
You can start a questionnaire from your dashboard by selecting 'Enter invitation code'.
Your customer may have sent you an invitation code for a specific questionnaire that needs to be completed. If you have not already completed this questionnaire for another customer that you just need to share, you can start a new questionnaire by entering the invitation code which will trigger the specific questionnaire for you.
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. Select 'Enter invitation code' located underneath the 'Get started' link on the Dashboard
3. Enter the invitation code that you have received
4. Continue responding to your questionnaire until completion

I have received an invitation code from my customer - What do I need to do?
The invitation code will be sent directly from your customer to either share an existing questionnaire you may have, or to start a new, shared questionnaire that you can complete.
The invitation code will allow your customer to have direct access to your data.
The SUPPLIERASSURANCE platform has been designed for you as a supplier to share and manage your data in a fast, secure and controlled way. The invitation code allows the customers to send direct invitations to you with specific requests.
Although you are able to 'Request' a share with your chosen customer, you would have to wait for these to be accepted. The invitation code allows for the data to be shared right away.
To start and share a new questionnaire with your invitation code:
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. Select 'Enter invitation code' located underneath the 'Get started' link on the Dashboard
3. Enter the invitation code that you have received
4. Continue responding to your questionnaire and submit this for validation
To share an existing completed questionnaire with your invitation code:
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. Use the search box in your dashboard to locate the existing, completed questionnaire by searching with location details
3. Once located, select 'Share'
4. On the 'Share' page, select the 'With invitation code' tab
5. Enter your invitation code in the provided field to share your questionnaire

Can I re-use answers from a similar previously completed questionnaire?
Yes, if you have previously completed another questionnaire you are able to re-use the answers for a new questionnaire you need to complete.
The questionnaires on the SUPPLIERASSURANCE platform are designed to cover a defined scope such as a location, a product or a contract. As such you may be required to complete multiple questionnaires for one or more of your customers. Where questionnaires cover policy and process information, you may want to re-use the answers from a previously completed questionnaire to save time and to ensure consistency of response. You can still edit any answers even if you use the re-use functionality in the SUPPLIERASSURANCE platform.
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. On your dashboard, locate the existing questionnaire you wish to complete or start a new questionnaire by selecting 'Enter Invitation code' on the right-hand side
3. Select 'Respond' and progress
4. Use the re-use answers from another questionnaire link when it is shown on the questionnaire introduction page
5. Select the questionnaire you wish to 're-use' the answers from and select 're-use'
6. Edit any answers that need to be changed for the scope of this new questionnaire before you submit it
7. Press 'Submit' on the final page of the questionnaire to submit your answers

I have an existing questionnaire. My colleague received a request to complete an SAQ for another customer. Do we have to complete another questionnaire?
No, you will not have to complete the questionnaire again if the customer invitation links to the same request.
The SUPPLIERASSURANCE platform allows you as a user to complete and share questionnaire results in a controlled, fast manner. As there are large organizations, we made it as easy as possible to complete requests for different locations in a fast way by re-using existing answers or sharing your answers for one questionnaire with multiple customers at one time, reducing the time you have to take to complete many questionnaires.
To share an existing completed questionnaire with your invitation code:
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. Use the search box in your dashboard to locate the existing, completed questionnaire by searching with location details
3. Once located, select 'Share'
4. On the 'Share' page, select the 'With invitation code' tab
5. Enter your invitation code in the provided field to share your questionnaire
To share an existing completed questionnaire by requesting a share:
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. Use the search box in your dashboard to locate the existing, completed questionnaire by searching with location details
3. Once located, select 'Share'

How do I download a questionnaire PDF report?
A PDF report of your questionnaire can be completed on the individual questionnaire's details page.
A PDF report of the questionnaire will become available once the questionnaire has been completed. The report will be available for download on the 'View' page of the questionnaire. Once downloaded, you are able to view the report which outlines the questionnaire answers that have been filled in.
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. Use the search box in your dashboard to locate the existing, completed questionnaire by searching with location details
3. Select 'View' on the questionnaire tile and this will take you to the questionnaire's details page
4. Scroll further down until 'Report' on the right-hand side and select 'Report' and you will be able to download a PDF file of your results

I have not received an invitation code - How can I share my questionnaire?
You are able to 'Request a share' with a chosen customer for them to accept. Once the request has been accepted, your data will be shared.
The SUPPLIERASSURANCE platform has been designed for you as a supplier to share and manage your data in a fast, secure and controlled way.
You can 'Request' a share with your customer who can either accept or decline your request and once accepted, your customer will have access to your data.
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. Select 'Start a new SAQ 5.0' and progress to complete the questionnaire
3. Once you have submitted your questionnaire for validation, return to your dashboard
4. Locate the questionnaire you now need to 'request a share' for
5. Select 'Share'
6. On the 'Share' page, select the 'Without invitation code' tab
7. Use the search option or scroll further down to locate the customer you wish to share your questionnaire with
8. Select 'Request' a share

How do I share my completed questionnaire answers with my buyer?
You can share your answers in two ways, with an invitation code or by requesting a share for your customer to accept.
The SUPPLIERASSURANCE platform gives users and customers the ability to share their data in a secure and quick manner. If a customer invites you as a supplier, an invitation code will be sent to you directly. This invitation code can be used to start a new questionnaire, or to share existing questionnaire answers with your customer right away.
Should you not have received an invitation code, you are able to 'Request a share' with your selected customer which will need to be responded to and accepted by them.
To share an existing completed questionnaire with your invitation code:
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. Use the search box in your dashboard to locate the existing, completed questionnaire by searching with location details
3. Once located, select 'Share'
4. On the 'Share' page, select the 'With invitation code' tab
5. Enter your invitation code in the provided field to share your questionnaire

Once I submit my questionnaire, who can see my answers?
Users on your own account with the 'Viewer' permission and your customers that the response has been shared with can see your answers.
Your questionnaire and the answers that you provide are considered confidential, as defined in the terms and conditions. Your customers will only be able to see the results of your questionnaire, as well as the documents that you decide to share with them. Different user permissions will also help you to ensure that only selected users can edit and share data.
To review your users' permissions follow the below steps:
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. On the dashboard, select the cog, shown next to your organisation's name
3. On the right hand side, select 'Manage your users'
4. The screen now will show you all existing users and to view their permissions or change them, select 'view'
To review which customer has access to your questionnaire answers:
1. Sign in to your account on the SUPPLIERASSURANCE platform
2. Select 'Manage data sharing' on the right hand side
3. Select the 'Data you are sharing' tile and search for the questionnaire required by using the search filter

Contact details of upstream suppliers refers to the name, postal address and email address of all suppliers in your product supply chains.

Relevant commodities contained within or used to make components refers to the relevant commodities listed in Article 2 and Annex I in the Regulation (EU) 2023/1115 (cattle, cocoa, coffee, oil palm, rubber, soy and wood)
HS codes of relevant commodities contained within or used to make components refers to the Harmonized Commodity Description and Coding System, also known as the Harmonized System (HS) of tariff nomenclature is an internationally standardized system of names and numbers to classify traded products. The HS code consists of 6 digits. The first two digits designate the Chapter wherein headings and subheadings appear. The second two digits designate the position of the heading in the Chapter. The last two digits designate the position of the subheading in the heading.
Common name of tree species of relevant wood commodities refers to native names given to a species based on local language or culture.
Scientific name of tree species of relevant wood commodities refers to the universally recognized names assigned to each species using a standardized system called binomial nomenclature.
Quantity of relevant commodities sourced refers to the amount or volume of relevant commodities listed in Annex I of Regulation (EU) 2023/1115 that are sourced by your company. In the case of operators or traders, companies must also monitor and document the amount of relevant commodities they import into or export from the EU market.
Date of production of relevant commodities refers to the date on which the relevant commodities were harvested.
