A Self-Assessment Questionnaire can provide an insight into a supply chain whilst identifying potential risk areas for improvement. It is important for companies and suppliers to meet the expectations of law and regulation.


A Self-Assessment Questionnaire could contain general company information to understand facility information, a workforce profile and the employment relationship. It should include environmental information of management systems and training, greenhouse gas emissions, energy usage, water and waste management. A Self-Assessment Questionnaire could consider social elements such as workplace management, health and safety, child labor and young workers, discrimination, freedom of association and collective bargaining. It could also self-validate a company's governance regarding accountability, grievance, supplier management, stakeholder engagement and disclosure.


1. Go to your dashboard on SupplierAssurance
2. Find the questionnaire you have been working on by using the search functionality
3. Select 'Respond' and progress to reference 15c by selecting 'Next'
4. Select 'Yes' and select 'Choose File' and upload your Self-Assessment Questionnaire
